Resident Life at Hamilton Home
When we reach the stage of our life when we need a little more help, it should never be seen as a bad thing. In fact, in most cases it is the beginning of a whole new set of memories, laughs and smiles. Hamilton Home prides itself in providing the highest level of care and resident life. We are different from most facilities as we are a mid-sized facilities with only 18 residents. Each member of our staff is familiar with each resident’s care plan and specific needs. Cookie-cutter programs in large facilities are not as personalized as what we are able to offer. Resident’s at Hamilton Home enjoy quality care by trained staff in a home like setting.
Services Provided
Below is a sample list of our basic services, care plan levels and optional ad-on services. The administrator and executive director will collaborate with both the family and the resident to develop a care plan and ensure the resident’s needs are met. Every resident is different and needs an individual care plan tailored to their specific needs. The care plan levels below are not concrete, but are used as a basis for determining the level of care needed by a particular resident. We use a points system to help evaluate the level of care needed by the resident. Points are evaluated based on the level of need for each of the following activities of daily living. As the administrator and caregivers observe the resident over the first month at Hamilton Home, the care plan may be modified to accommodate for a residents current needs. Basic Services are those provided to every resident apart from the individualized Care Plan. These services are covered in the base monthly rate.

Independent Resident
Our level 1 plan is designed for residents that are most independent with their self care but need to live in a safe community with 24 hour supervision, meals, and medication management.
A resident in this category needs minimal care and supervision by our staff in ALL their activities of daily life (ADL’s). They can go to the bathroom on their own without a caregiver present. They may have an occasional accident (no more than 1 per week) that requires minimal assistance by a caregiver. They can shower on their own without a caregiver present. They can shave, clean dentures, brush teeth, use lotion, put on deodorant with only friendly reminders from staff. They can dress themselves for the most part with occasional assistance by a caregiver with buttons, jewelry or hard to reach items. They can transfer in and out of bed and in and out of a chair without a caregiver present. They can adjust their body and pillow and comforter in bed without assistance. During the night, they only need occasional checks by the night staff and do not need assistance with getting up, going to the bathroom, getting back in bed, changing briefs, etc. They can feed themselves without caregiver assistance. During the day, they need minimal assistance from staff with TV channels, volume, participating in activities, specialized exercises or activity programs and assistance with other random tasks. All basic services are included in this level of care: Basic Medication Management, Weekly Laundry, Weekly Housekeeping, Basic Meal Preparation, Activities Programming and 24-hour supervision.
Basic Assistance
If your loved one is needs some assistance with dressing, showering or toileting, but can navigate the rest of their day on their own, our level 2 plan would fit best.
A resident in this category needs substantial assistance in 1 – 3 of their ADL’s and occasional supervision and/or assistance in the rest of their ADL’s. They may need help with showering, toileting and dressing but be independent in the rest of their ADL’s. They should be continent. They should be able to ambulate around the facility on their own, be independent at night other than assistance with the bathroom, be independent during the day other than assistance with toileting, dressing, and showering. They should be able to feed themselves, transfer in and out of chairs during the day and in and out of bed at night and adjust themselves in bed at nighttime. They should be able to attend and participate in activities without substantial assistance by staff. All basic services are included in this level of care: Basic Medication Management, Weekly Laundry, Weekly Housekeeping, Basic Meal Preparation, Activities Programming and 24 hour supervision.
Transfer Assistance
Our level 3 plan is designed for those residents that need help transferring safely and require assistance with many of their daily tasks. They are not resistant to the help the caregivers provide.
A resident in this category requires substantial assistance in 4 – 7 of their ADL’s and occasional supervision and assistance in the rest of their ADL’s. For example, a resident in this category might need a caregiver to be present and assist with all toileting needs, grooming, dressing and showering. They may be incontinent. They may not be able to get around on their own and may need help moving about the facility, either in a wheelchair or having someone walk by their side while they use a walker. They may need help with transfers in and out of bed and in and out of a chair. They should be able to adjust themselves in bed to be comfortable without caregiver assistance. They should be a minimal fall risk and should have mild cognitive impairment at most. All basic services are included in this level of care: Basic Medication Management, PRN Laundry, PRN Housekeeping, Basic Meal Preparation, Activities Programming and 24-hour supervision.
Extensive Assistance
A resident in this category requires substantial assistance in 8-10 of their ADL’s. They should still be able to assist with some tasks and be able to complete some ADL’s on their own. In addition to assistance with toileting, grooming, showering, dressing, transferring and ambulating as mentioned above, a resident may need assistance with feeding and drinking fluids and possibly special meal preparation. They may require two caregivers to assist them with transfers or walking. They may have a dementia diagnosis and present with more confusion and disorientation. They may need assistance participating in facility activities and with personal tasks such as phone calls, letters ect. They may be on hospice and have more intricate medications regimens or complex medical needs such as catheters, wounds, oxygen/breathing treatments, ect. During the day and night, they may need extra assistance from staff to ease anxiety or assist with confusion, more time and special preparation of their medications, or possibly one-on-one help with PT exercises and strength training. All basic services are included in this level of care: Complex Medication Management, PRN Laundry, PRN Housekeeping, Complex Meal Preparation, Special Activities Programming and 24-hour supervision.
Total Care/Bedridden
A resident in this category needs full assistance with all ADL’s. They are unable to do anything on their own anymore. They may have dementia and need constant supervision to prevent falls or harm to themselves or others. They may need frequent bed changes and rotations to avoid wounds. They may be a 2 person assist or need hoyer lift transfers. They may also be resistant to our assistance with ADL’s and taking their medications or they may have complex medical needs. They may be on hospice. All basic services are included in this level of care: Complex Medication Management, PRN Laundry, PRN Housekeeping, Complex Meal Preparation, Special Activities Programming and 24-hour supervision.
